I have read and understand the conditions of the scholarship as explained in the scholarship criteria. I affirm that I plan to pursue a High School education. I give permission to officials of my institution to release transcripts of my academic record and other information requested for consideration in the Van’s Joy Foundation Scholarship committee. I understand that this application will be available only to qualified people. I waive the right to access letters of recommendation written on my behalf. I also agree to photography/videography in conjunction with the scholarship award. I hereby grant Van’s Joy Foundation permission to use my likeness in photographs and/or video in all of its publications, including web space, social media, and in all other media, whether now known or hereafter existing, controlled by Van’s Joy Foundation. I affirm the information contained herein is true and accurate to the best of my knowledge and belief.
